Cabin Creek Health Systems is Federally Qualified Health Center (FQHC) located in Kanawha County, WV.  Cabin Creek provides primary care as well as many other health services to people in both urban and rural setting through multiple locations throughout the county. Cabin Creek Health Systems consists of the following locations/Services:

  • Dawes, WV – Cabin Creek Health Center & Pharmacy (Also contains Pulmonary Rehab Facility and Black Lung Clinic)
  • Belle, WV – Riverside Health Center (Located in Riverside High School with School-based Health Services)
  • Kanawha City, WV – Kanawha City Health Center & Pharmacy
  • Charleston, WV – Sunnyside Health Center (located in the Kanawha County Health Department)
  • Sissonville, WV – Sissonville Health Center & Pharmacy (Also contains full service Dental Clinic)
  • Clendenin, WV – Clendenin Health Center & Pharmacy (Also contains full service Dental Clinic)

Cabin Creek Health Center also offers School-base Health Services in 3 Kanawha County high schools and 3 Kanawha County middle schools. Medication Assisted Treatment Programs (MAT) are operated at 3 of the health centers with a stand-alone MAT facility in Kanawha City.

The Mission of our Advanced Practice Provider Primary Care Fellowship is to develop highly trained primary care nurse practitioners and physician assistants who are lifelong learners practicing evidence-based, individualized, and compassionate care that meets the unique health needs of our underserved and vulnerable community.

Applicants must be:

  • Recent graduates (less than 18 months) from an accredited Masters or DNP program
  • Licensed and credentialed (or license and credential eligible) in WV as a PA, FNP, AGPCNP prior to the start date of the program

The next cohort will start September 9th, 2024. Applications will be accepted and reviewed on a rolling basis. Applicants are encouraged to apply as soon as possible to be considered. Spots in the program will be offered on a space-available basis. The program will continue to accept and review applications until the class is filled but no later than August 1st.
